Background and Production

The series follows the life and work of Kensuke Goto, a dedicated physician providing medical care on a remote Japanese island. Although the character's name is written as "Goto," the local islanders pronounce it "Koto," which serves as the basis for the series title. The 2004 special expands upon the narrative established in the 2003 series, featuring storylines that include the character Shinichi Mikami.
