Dr. Koto's Clinic 2004 Autumn (Japanese: Dr.コトー診療所2004) is a two-part Japanese television drama special that aired on Fuji TV on November 12 and November 13, 2004. The production serves as a continuation of the popular medical drama series based on the manga Dr. Koto Shinryojo by Takatoshi Yamada.

Plot

The story follows Dr. Kensuke Koto, a physician working on the remote Shikina Island. Although his name is officially Goto, the local islanders pronounce it as "Koto," which gives the clinic its name. One year after his arrival, the clinic—managed alongside staff members Ayaka Hoshino and Wada—has become an essential part of the community. As the island prepares for a festival to pray for a successful fishing harvest, the clinic faces a surge in emergency cases, testing the team's resolve. Amidst these professional pressures, personal developments arise, including discussions regarding the potential marriage of Ayaka and Dr. Koto.
